1. Undergo a COVID-19 test prior to travel to Uganda and to willingly disclose the results there-from to both the carrier (airline) and to the Government of the Republic of Uganda;
2. Undergo a COVID-19 test on arrival in Uganda;
3. Undergo mandatory quarantine for a minimum period of 14 days at a facility approved by Government of Uganda;
4. Fully comply with the contents of this undertaking;
